The needs of the elderly are varied, and can be complex. Although many live independently well into old age, others require more attention and assistance. AgeQuest offers a combination of expertise in issues of aging and a caring, personalized approach. We establish positive working relationships with elders and their caregivers, providing information, encouragement, and hands-on help when needed. Examples of the services we offer are listed below.

Comprehensive Assessment – Identification of individual strengths and weaknesses is essential when developing a plan for maximizing independence and defining options. AgeQuest provides a skilled and comprehensive assessment of health status, functional ability, home safety and resources. A summary is presented, and a service plan developed in keeping with the priorities, values and preferences of the senior and key caregivers is developed.

Care Coordination –  Once needs and desired services are identified AgeQuest can put the service plan into action, making needed referrals, monitoring care, and maintaining close contact with the senior and caregiver. Response to the plan and ongoing needs are continually evaluated.

Counseling – Some families opt to coordinate care themselves, once needs are identified. Staff from AgeQuest are available to provide information, support and recommendations on a schedule determined by the caregiver.

Institutional Care Monitoring –  AgeQuest can provide skilled monitoring of seniors in supportive care settings, including retirement communities, assisted living, residential care and nursing homes. This is often helpful when family members live out of town, or when a guardian or conservator is responsible for overseeing the care of an individual.
